Got Stuck in multiselect dropdown menu

I realized a nice and simple webapp for collecting data from Modellist to partecipate in Modelling Contest and everything is ok but…how about a multiselect choiche from user ? Can i insert a multiselect dropdown menu inside an existing form ?
