Phone is buggy but the preview is not

Hey all,
I have a testflight build and a preview that are running the same update. Yet for some reason the phone is not completing the form correctly, even though the preview is.

Here you can see what the action is meant to do:

Its suppose to trigger an event to make, and the preview does work correctly, where the new records created are linked to the organisation. But the testflight version does not.
Why??
