Creating a custom form

Hiya guys, I’ve been trying my hand at creating a custom form as I’ll like to add a dropdown list to the one I’m creating. Unfortunately I unable to find any info on how to do so properly. When I hit the custom submit button to create the entry, it doesn’t. Any info/help would be greatly appreciated.

Please ignore, I’m new and just finding my way around. Figured it out with the regular form.
