How to make a Poll?

Has anybody made a questionnaire with the following:

  1. Each list item has a question and options attached to it. (say Yes or No)
  2. Each User can answer either select YES or NO.
  3. These responses get stored in the DB and can be used to show the poll result to the users.

Any help is appreciated
