Create a numbered drop down list

Hi Team, I am trying to create a basic numbered drop down list (1,2,3,4,5,6) and once the user selects say 3:

Next app screen will list 3 labels and below a search button (text box style, as if you were typing an address).

Each label will be Me, Person 2, Person 3. With a search box below each label.

I hope this is clear?
