Customizing form elements like adding a text element in between and 2 columns form?

Hi! I have been creating a form to let users filter out what they want to see on the list on the next page.

I created a separate database to store what users want to see, basically similar to [upper price limit], [lower price limit], [fruit1], [fruit2] etc…

Is it possible to show the form like this? Adding a text element in between the fields and even 2 columns inside the form?
