Create or Load existing data

Hi,

I have a simple task of creating or loading a form based on two criteria – if a record already exists with the same name and date then load ELSE create. I tried to accomplish this using two filters in the “create new record” action but something is off.

It never loads the correct data and half the time creates a new record even if a matching record already exists.

Can anyone suggest what I am doing wrong here? Images attached.

Input 2 is a hidden input field that is set based on what button the user clicks on.

Stacked two buttons … one for create with create action and one for load that was converted to a list with filtered list based on multiple criteria.
