Welcome to the community 
This is easy! All you need to do is add a list ( Custom List, Simple List etc ) and connect the list to the collection that these information stored and rename the title as Subject from magic text | Grade from magic text | weight from magic text and you are done!
@ghislain posted a app too that this is there with a Simple List. Thanks ghislain for that!
